We are looking to recruit a Level 1 Social Worker to be based in our East Locality Integrated Care team within the Wakefield District.
37 hours on Rota over 7 days to include weekends and evenings
The team work with people aged 18 and over, including older people and those with a physical disability.
The team consists of social workers, physiotherapists, occupational therapists, care coordinators, community nursing staff and Third Sector.

As a qualified Level 1 Social Worker, you will play an important role in helping vulnerable people live as independently as possible. Taking responsibility for assessments and identifying where vulnerable adults may be at risk, we are looking for a qualified Social Worker with up-to-date professional expertise and experience in case management and support planning. In addition, it's important you are an organised professional that has extensive current knowledge of all relevant legislation.
Applicants for this role must be qualified to a Degree level or equivalent and be registered as a Social Worker with HCPC registration. This is an essential requirement for this role.
The successful applicant will be required to complete a Disclosure Application Form and to provide criminal conviction information.
